New Patient Registration

How do I become a patient?

We can only register you and your children if you live in one of the areas we cover. The areas are: Central Milton Keynes (MK9), Bradwell Common (MK13), Heelands (MK13), Oldbrook (MK6), Conniburrow (MK14), Bradwell Village (MK13), Campbell Park (MK9) and Loughton (MK5). The Practice does not register patients who live out of our area.

 

We have an information sheet which explains what documents you need to register with the Practice. The information sheet is available from our reception staff and on the link below. You can register any time between 10.30 and 15.00 Monday to Friday.

When you arrive at reception to register you will be booked into an appointment time with the registration clerk.

Please ensure you have your NHS number when registering.

What forms do I need to complete?

You can either pre-register on line by clicking on the tab above and following the links, or you can download and complete the Registration Form  and bring it into the surgery with your documents.  Please let us know if you have completed an online form when you bring in your ID.

What happens when I become a patient?

You will be given or sent a letter confirming your registration and details of how to access online services. You will also be given a registration card which gives the name of your registered Doctor who will have overall responsibility for the care and support that the Practice provides to you. This does not prevent you from booking an appointment with another GP in the Practice. The registration card also gives contact details for arranging and cancelling appointments.

All new patients are asked to complete a simple medical questionnaire. We use the information in order to provide care and support to you while we request your medical records.  

 

You will also be given details of our Patient Participation Group which works with, and supports, the Practice to improve services for all patients.
